Dr. Konrad Dias, PT, DPT, PhD is an American Physical Therapy Association Board Certified Cardiopulmonary Clinical Specialist with Emeritus status and currently serves as Professor of Physical Therapy at California State University in Sacramento, California. He received his PhD in Physical Therapy from Nova Southeastern University, FL and his DPT degree from the University of Massachusetts. Dr. Dias has strived to maintain an advanced record of teaching and scholarship by disseminating contemporary physical therapy practice both nationally and internationally in several countries across the world. He currently serves as Chair of the Research Committee for the APTA Home Health Academy.

Bridging Evidence and Practice: A Structured Practical Framework for Heart Failure Rehabilitation

As the global prevalence and clinical burden of heart failure continue to increase, there is a critical need to delineate and implement evidence-based rehabilitation strategies that optimize health outcomes, enhance quality of life, and attenuate disease progression. This session will present information from the latest heart failure clinical practice guidelines along with a novel, five-step knowledge translation framework designed to support physiotherapists in the comprehensive management of individuals with heart failure.
The targeted ABCDE framework integrates key clinical domains: (A) Assessment of Stability, (B) Behavior Modification, (C) Cardiorespiratory Fitness Testing, (D) Dosage of Interventions, and (E) Education. Each component reflects current evidence and clinical practice considerations relevant to safe and effective rehabilitation delivery. Through systematic examination of the conceptual and practical foundations underpinning each domain, participants will develop the capacity to translate evidence into practice and adopt a structured, theory-informed approach to heart failure management.
